Pen-Pych (386m), UK - hike, taking selfie


A teenager died in a freak accident while posing for a photo on a mountain ledge, an inquest heard. Fynley Jones, 18, was walking with school friends when they stopped to take selfies at the top of Pen-Pych mountain

But tragedy struck when Fynley "lost his footing" as he posed on a ledge - and he plunged down a "considerable drop" as his horrified friends watched. An inquest heard emergency services rushed to the remote scene in the Rhondda Valleys. Paramedics battled to save the teenager but he died from his injuries that day.

A post-mortem examination revealed Fynley, who loved mountaineering, surfing and skateboarding, died of a "blunt head injury." The A-Level student, of Pentre, had been due to start studying Artificial Intelligence and Robotics at Aberystwyth University after securing a place.

Assistant coroner Rachel Knight said: "Fynley had been together with a group of school friends, taking photographs at the top of the mountain. Fynley decided to pose for photographs on a ledge which was precarious, and sadly, he lost his footing and fell. The fall was down a considerable drop, and Fynley sustained fatal injuries."

The coroner recorded a conclusion of accidental death at the inquest in Pontypridd Coroner's Court.

Raven Fall, Indian Canyon, Utah - ice climbing, falling ice

  A 41-year-old ice climber named Meg O'Neill, also known as "granitenomad" on Instagram, died while climbing with friends on Raven Fall near Indian Canyon. O'Neill was struck by falling ice after pushing her climbing partner out of the way. Reports say that a 21-year-old female was belaying a 34-year-old male on lead when the ice collapsed. The climber fell 40 feet and suffered serious injuries, but the belayer was left physically unharmed thanks to O'Neill's heroic act. Ben Nevis (1,345 m), Scotland - mountaineering, avalanche

  A climber has been killed in an avalanche on the UK's highest mountain - the first such white death in Scotland in nearly three years. The 49-year-old man was with a fellow climber on Ben Nevis when they plunged nearly 2000 feet on the 4413ft high mountain's North Face. The other climber - who is 42 - miraculously survived but suffered serious injuries. Nearly 40 rescuers from  Lochaber  and Glencoe mountain rescue teams were sent after the alarm raised around 3.30pm on Friday. The pair - both UK based - were were a group of four on Number Two Gully, and the other two raised the alarm. But Donald Paterson, leader of Lochaber Mountain Rescue Team, said the initial information was "patchy". "It appears they were near the top and their weight made a cornice (an overhanging ledge of snow) give way and that triggered the avalanche," he said.
